Abstract
Primary debulking surgery is besides the systemic treatment the most important therapeutic strategy in ovarian cancer . Till now the surgical management does not differ for different histological subtypes . Primary surgery with the aim of macroscopic complete resection is standard of care . Neoadjuvant management is no standard of care and should only be investigated within clinical trials . Postoperative residual tumor after primary surgery is the most import prognostic factor besides tumour stage and is the only prognostic factor that can be influenced . The LION trial has shown the high impact of prospective, randomized surgical trials . Conclusion of the LION trial is that in advanced ovarian cancer with clinically non-suspicious lymph nodes no longer prophylactic systematic lymphadenectomy should be performed .
