Therapieoptionen bei der saisonalen (intermittierenden) allergischen Rhinitis

Abstract

Introduction: The treatment of allergic rhinitis mainly includes allergen avoidance measures, pharmacotherapy, allergen immunotherapy (AIT), and non-pharmacological measures. While allergen avoidance is often inadequate, pharmacologic therapy and AIT act synergistically and are by no means either/or alternatives. Supportive non-pharmacologic measures can be used in addition. Methods: A literature search was performed to analyze therapy options for allergic rhinitis and to determine the available evidence by searching Medline, Pubmed, and the national and international trial and guideline registries and the Cochrane Library. Human studies published on the topic in the period up to and including 08/2023 were considered. Results: Based on the international literature and previous experience, recommendations are provided. Medications used in the pharmacotherapy of AR primarily include intranasal corticosteroids, intranasal and oral antihistamines, leukotriene antagonists, intranasal cromoglicic acid preparations, intranasal and oral vasoconstrictors, and nasal irrigants. Intranasal glucocortico-steroids are the first-line therapy in patients with intermittent and persistent allergic rhinitis, and AIT is the only causal therapeutic option. Conclusions: The future of seasonal allergy therapy will benefit greatly from personalized medicine, starting from precision diagnosis to personalized therapy considering consistent biomarkers. Simple practical problems also need to be solved such as low adherence to pharmacological therapy and allergen immunotherapy.
